OBTAIN A REPEAT PRESCRIPTION?

To avoid errors, we do not take telephone requests. You can either bring your request and put it in the appropriate box on the reception counter, post, fax or email it to us.

Alternatively this can be done via Emis access.

Tick the required medication on your prescription counterfoil. Indicate your preferred chemist if you would like the prescription collected and made ready. Please allow at least 48 hours to prepare your prescription.

Do You Pay For Your Prescriptions?

Prescriptions are free for: men and women over sixty, pregnant women, children under sixteen (or nineteen in full-time education), mothers of babies under twelve months, people who have low income, people who receive income support (check list on Form P11 from Social Security or Post Office), people with family credit and people with certain chronic conditions. If you are not entitled to free prescriptions a 'season ticket' may save you money. This is a prepayment certificate. If you need more than five items during four months or more than fourteen prescribed items during the year, you will save money by buying one.

Sick Notes

Certificates for sickness for periods of less than a week should be signed by the patient themselves, using form SC2 available from the surgery or your place of work. A medical certificate from the doctor is not normally required. If your employer insists on the production of a medical certificate for less than a week, a fee will be charged. Sick notes can be backdated to the first day of an illness and do not require a ‘same day’ appointment.
